Using the MEDIQWK brochure as draft guide, go to the MEDIQWK web site and key in your emergency medical data. Submit the completed electronic form. MEDIQWK then send you a confirmation of the data and a card kit.
MEDIQWK provides you with a membership card with your name and an access code. The MEDIQWK Card will provide your emergency medical data and references to your doctors and dentist in the most efficent and quick way.
The MEDIQWK Card instructs the medical personnel how log into the MEDIQWK system (QR "quick response" scan devices will log in automatically) and use the access code to produce your emergency medical profile. They cannot change your data. Only you can change your data.
Included in the card kit is your personal access code. This private code is to be used by you to update your data if your medical status changes.
An option for your card kit is the Personal Decal (temporary tattoo) kit. This will allow you to place the MEDIQWK Card data directly on your person if participating in sporting or travel situations where the MEDIQWK Card could not be carried. You determine when and if to use this feature. This way you are never alone and separated from your emergency medical data.
